Die Saponine sind für die oft beschriebenen Verdauungsstörungen verantwortlich.
Die Pflanze enthält etwa zwanzig Herzglykoside, von denen Convallatoxin und Convallatoxol die wichtigsten sind. Diese Substanzen wirken sich auf den Herzmuskel und die Durchblutung aus, ähnlich wie die Herzglykoside im Fingerhut (Digitalis purpurea). Die unüberlegte medizinische Verwendung der Mayflower oder Lily of the Valley ist gefährlich. Der Abstand zwischen gewünschten Wirkungen und toxischen Nebenwirkungen ist manchmal sehr eng.
